Nurse (Veteran's Health Education & Health Promotion Disease Prevention) at Veterans Affairs, Veterans Health Administration in Boise, Idaho, United States Job Description Summary The Boise VAMC Veteran's Health Education (VHE) and Health Promotion Disease Prevention (HPDP) Coordinator is responsible for programmatic management to ensure a coordinated approach to planning, delivering, and evaluating evidence-based Veteran-centered health education programs, services, and resources materials for Veterans and , as appropriate, their family members or caregivers. Responsibilities VA offers a comprehensive total rewards package: VA Nurse Total Reward Duties may include, but not limited to: Completing national NCP-directed Healthy Living Team Self-Study Orientation Program within 90 days of hire or appointment to the position and the VHA Central Office-directed HPDP Program Manager Self-Study Orientation Program within 60 days of hire or appointment to the position. Planning, developing, implementing, monitoring, and evaluating the overall VHE/HPDP programs at the facility and all CBOCs. Ensuring that VA medical facility VHEI/HPDP programs and services are Veteran centered. Leading VA medical facility efforts for VHA-wide Veteran-centered health education programs, products and resources including new patient orientation, the Veterans Health Library (VHL), self-management programs and other interventions and initiatives that induce health education. Conducting a VA medical facility-wide assessment of Veteran-centered health education programs and HPDP services to determine the gaps or needs, services and products for specific Veteran populations or diseases and conditions, at least every 2 years. Leading the VHEI/HPDP strategic planning process and integrating into the VA medical facility strategic plan and related programs' strategic plans. Program strategic plans are to be developed at least every 2 years and aligned with VA medical facility strategic plans. Participating in orientation programs for new VA health care clinicians, to ensure they are knowledgeable about the VA medical facility VHEI/HPDP program, can effectively refer Veterans, family members or caregivers, if appropriate, to VA medical facility Veterans health education programs and services and document, in the medical record, the Veteran education content, services and programs that they provide in accordance with VA medical facility documentation and The Joint Commission requirements. Collaborating Closely with other members of the Healthy Living Team and other VA health care providers to integrate VHEI/HPDP programs and resources across the VA medical facility continuum of care. Partnering with other clinical disciplines and service lines (e.g., Primary Care, Specialty Care, Acute Care, Geriatrics and Extended Care, Mental Health and Pharmacy) and functioning as the designated subject matter expert and advocate for implementing VA medical facility VHEI/HPDP programs and activities. Coordinating with VA medical facility staff (e.g. Public Affairs Officer, VA Voluntary Services Coordinator, My HealtheVet Coordinator) to develop and available listing of VHEI/HPDP programs and services and promote VHEI/HPDP programs and services to Veterans via VA medical facility newsletters, websites, outreach events, educational fairs or other channels. Serving as the lead facilitator of the TEACH Program and supporting Veteran centered communication skills training and clinician coaching for clinical staff. Chairing the VA medical facility VHEI Committee or subcommittee. Leading or co-leading the facility or HCS HPDP Program Committee or subcommittee. Representing the VA medical facility VHEI/HPDP program on related VA medical facility committees. Serving as the point of contact for communication between the VA medical facility, VISN (including the VISN leads) and national VHEI/HPDP programs. Providing feedback and reports on VHEI/HPDP implementation and programming as well as sharing strong practices as requested to the VISN Program Leads. Providing feedback and reports on VHEI/HPDP programs, services and products as requested to the VA medical facility Director. Providing leadership in the development or modification of delivery processes to support the successful implementation of evidence-based HPDP services into clinical care. Preferred Experience: Applicant must be confident with Microsoft Word, PowerPoint, and Microsoft programs in general. Applicant must have excellent presentation skills, both verbal and visual. Other preferred experience includes chairing a committee, timely communication, the ability to collaborate across many different environments/disciplines, systems thinking, and organizational stewardship skills. Ability to create health education materials, both in paper and electronic forms, at the health literacy level appropriate to the Veteran population a plus. Qualifications Basic Requirements: Citizenship: Citizen of the United States in accordance with section 7402 to title 38, United States Code (U.S.C.). Non-citizens may be appointed when it is not possible to recruit qualified candidates in accordance with 38 U.S.C. 7407(a). English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f). Education: Graduate of a school of professional nursing. (1) Education Accreditation Requirement: Nursing education must be from a school of professional nursing accredited by one of the following accrediting organizations at the time the program was completed by the applicant: (a) The Accreditation Commission for Education in Nursing (ACEN), 3343 Peachtree Road NE, Suite 850, Atlanta, GA 30326. The ACEN accredits all levels of nursing programs, including clinical doctorate, masters/post master's certificate, baccalaureate, associate and diploma nursing education programs.
