Expert level of clinical decision making based upon Benner's No vice toExpert Nursing Theory. Systematic processing of patient acuity an dtriaging of multiple demands.Provide guidance to internal and external customers to facilitate safepatient and operational needs. Anticipate po tential safety concerns andidentify high risk patient situations and int ervene or escalate asneeded.Engage in excellent written and oral communi cation skills with a focuson professionalism and customer service. Excel lent phone etiquette.Navigate complex systems and processes to maintain patient centeredcare.

Collaborate with multiple members of a care team (internal andexternal) to facilitate patient care needs.Communications, coordinating and scheduling tests and pre-procedures toprepare patients for Electrophysiology procedures (EP). EP is aspecialized field within c ardiology managing the electrical conductivityof the heart utilizing com plex tools and mapping systems. Examples of EPprocedures include: pacema kers, defibrillators, ablations, Watchmenimplants, etc.Telephonic Care M anagement to ensure safe patient centered care.Assessment of cardiac pat ients via phone, triage of care, schedule andprovide education to patien tsfor cardiac procedures. Duke Heart Centerhas developed a standardized process for our patients to contact us toseek and receive medical care w ith Duke Cardiology Call Center as theinitial point of contact. This car eisdelivered in collaboration withProviders, Clinical Staff and Duke Ca rdiology Call Center. Our patientscan request medical advice via telepho ne and through My Chart.Scheduling and coordinating procedures to includ e: Dx & InterventionCardiac Cauterization, Left Heart Catheterization, R ight HeartCatheterization, TAVR, Mitra-Clip, Heart Biopsy, Pre-Lung Tran splantEvaluation, Balloon Pulmonary Angiography, Peripheral Arteriograms , ASD& PFO Closure, etc..Transitional Care, follow care of patients from hospital to homeassessing this hospital discharge, medication reconcilia tions andproviding continued patient education. Nurses support patientsb y phoneto navigate care transitions as well as interact with providers,c linics, schedulers, home health and others to ensure that our patientsun derstand the discharge plan and are successful.Manage Prescription Refil ls for all DUH Cardiologist.Perform other related duties incidental toth e work described herein.herein.

Must have current or compact RN licensure in the state of North Carolina. BLS required.
