Job Description
As a Patient Advocate at Ovation, you will serve as the voice of hospice patients, ensuring their rights and dignity are respected as they navigate the end-of-life journey. You will offer invaluable assistance in understanding complex medical information, making informed decisions, and accessing the necessary support services.
ResponsibilitiesAct as a liaison between patients, families and the hospice care team to facilitate open communicationAdvocate for patients' needs, preferences and advance directives regarding treatment and careProvide emotional support and guidance to patients and loved ones during this difficult transitionEducate patients and families on their rights, hospice policies and available resourcesInvestigate and resolve patient grievances in a timely and professional mannerMaintain accurate documentation and comply with all regulatory requirementsCollaborate with the interdisciplinary hospice team to deliver holistic, patient-centered care
QualificationsBachelor's degree in healthcare, social work, counseling or related fieldMinimum 2 years of experience in patient advocacy, hospice or palliative care preferredStrong understanding of end-of-life care, patient rights and advance directivesExcellent verbal and written communication abilitiesCompassionate nature with outstanding interpersonal skillsProblem-solving skills and ability to mediate conflictsProficiency with electronic medical records
