Job Duties as a Transitional Care Coordinator:
Develop relationships specific to area to assist in maximizing censusIncrease referral rate from area hospitalsCoordinate post-discharge care services and resources, including home health services, durable medical equipment and follow up appointmentsProvide education to patients and families on post discharge instructionsServe as a liaison between patients, families, and healthcare providers to resolve issues and overcome barriers to careMaintain accurate and up-to-date documentation of transitional care plans, patient education activities, coordination efforts, and outcomesMonitor and evaluate the effectiveness of transitional care interventions, making adjustments as needed to optimize patient outcomesMaintains knowledge of current regulations governing Medicare, Medicaid InsuranceEmbrace company's core values and incorporate them into daily jobDemonstrate respect and compassion in every interactionPreform other Duties as assignedConduct comprehensive assessments of patients medical, social and support needs to develop individualized transitional care planQualifications for Transitional Care Coordinator:
2 years of relevant experience in Health Care Marketing or Business DevelopmentBachelors DegreeMust be 18 years or olderAbility to deal tactfully with personnel, residents, family members and the publicExceptional people skillsExcellent verbal, written and interpersonal communication skillsAbility to work independentlyMust have good time managementAbility to prepare accurate reportsMust be proficient in the use of a computerHave knowledge of healthcare systems, community resources and discharge planning processesMust have empathy, compassion and patient centered approach to support patients in transitions of care
