YMCA International Services is a unique branch of the Y of Greater Houston and seeks to meet the needs of Houston's significant refugee and immigrant communities. We provide comprehensive programs that focus on building human assets and fostering self-sufficiency. The goal of the program is to promote the health and wellbeing of eligible refugees by providing opportunities to increase health literacy, peer support, and wellness groups. This group is arriving with immediate and long-term physical and mental health needs, and it is critical to ensure they can understand and access the U.S. healthcare system. In addition, supporting non-clinical services for refugees are vital to enhancing the wellbeing of this population. As our Refugee Health Promotion Community Health Worker, you will collaborate with an Outreach Specialist and 4 program facilitators to implement the Health Education programming for the ARHP grant.

Duties /Responsibilities

• Build rapport and establish trust with RHP participants to provide health education and support and encouragement to access healthcare independently.

• Provide ongoing follow-up for those requesting individualized attention for their healthcare needs through motivational interviewing and goal setting

• Collaborate with partners to deliver health education programming to 13 unique peer support groups

• Create both video and written materials on health system navigation and various healthcare topics.

• Record daily activities for both groups and individuals through case notes in RMS

• Assess and analyze pre and post-tests as well as progress of individual clients accessing healthcare through self-reporting of clients and medical records, if available.

• Provide information about health topics and health systems in culturally relative way and communicate these matters with subcontractors and other partners.

• Act as a patient advocate and liaison with the agency, medical institution, and other social service agencies.

• Update the Program Director with monthly statistics

• Comply with the agency's policies and standards. Reports suspicious and inappropriate behaviors and policy violations.

• Follow mandated abuse and incident reporting requirements.

• Meet timelines and deadlines related to supporting systems and employee compliance. Examples include but are not limited to Kronos Workforce Ready for review and approval of time sheets

• Other duties as assigned including airport pickup and food distribution rotation

Outcomes and Indicators

Assists during enrollments, peer group sessions, and outside of group sessions to meet the outcomes and outputs of the RHPCompiles statistics on pre and post-test results including rate of participation in health education and wellness activitiesFollow up with individual clients to ensure that each client can access medical services for themselves and their family outside of group sessionsRequirements

• Excellent written and verbal English communication skills including being comfortable speaking in public

• Knowledgeable about community resources appropriate to the needs of the participants

• Knowledgeable about medical terminology

• Computer skills with Microsoft Suite, databases, and office equipment

• Ability to work with diverse people including adults and teens

• Ability to work independently and with minimal supervision

• Understanding of the diverse community and its experience with the health system

• 2 years' experience in the health and/or social service field

• Proof of CHW certification preferred. Training provided upon hiring if not certified.

• High School Diploma required; Bachelor's Degree preferred

• Valid driver's license, clean driving record, automobile liability insurance, and a vehicle.

• Fluency (spoken, written, and reading) in Swahili, Kinyarwanda or Spanish is required
