**Become a part of our caring community and help us put health first**Humana Healthy Horizons in Virginia is seeking a Senior Community Management Professional who will be responsible for directing community engagement practices, facilitating, and planning Member and Community Advisory Panels, collaborating with contracted medical and ancillary service providers, Federally Qualified Health Centers, Social Service Agencies, and care managers with the responsibility to link our membership to services and supports critical to the improvement of health outcomes.+ Responsible for representation of Cardinal Care member's interest, including input in policy development, planning and decision-making.+ Responsible for engagement in community boards and committees such as the Community Health Worker Network.+ Works directly with the Equitable Population Health Leadership team to drive outcomes aligned with larger organizational goals and builds a robust member outreach program.+ Compares member demographic and social supports and services data to identify opportunities to address and fill service gaps with strategic community partners and service providers.+ Cultivates need-based partnerships with social support and service providers, supporting evidence-based programs with positive health outcomes.+ Positively positions Humana among local/community groups and coalitions, community health leaders, and public health service providers.**Use your skills to make an impact****Required Qualifications**+ **Must reside in the commonwealth of Virginia.**+ Bachelor's degree.+ Two (2) or more year's previous experience in public health, community engagement or member/patient outreach.+ Experienced in planning, project management, community engagement and development, and resource mobilization.+ Understands the fundamentals of effective community engagement specific to Medicaid members, including those with behavioral health needs and varied health literacy.+ Proven knowledge and understanding of access to care barriers that impacts the Medicaid population.+ Exercises professional judgment and demonstrates strong facilitation, conflict management and consensus building skills.+ Ability to prioritize, organize, and coordinate multiple projects simultaneously.+ This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.+ Must be able to travel throughout the commonwealth of Virginia.+ This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100/300/100 limits.**Preferred Qualifications**+ Bilingual or Multilingual English/Spanish, Arabic, Vietnamese, Amharic, Urdu or other - Must be able to speak, read and write in both languages without limitations and/or assistance. _See Additional Information on testing._**Additional Information**+ **Workstyle:** This is a remote position that will require you to travel.+ **Travel:** Up to 50% of the time to Humana Healthy Horizons office in Glen Allen, VA for collaboration and face to face meetings as well as field interactions with community-based organizations, members, and their families.+ **Typical Workdays and Hours:** Monday - Friday; 8:00am - 5:00pm _Eastern Standard Time (EST)._+ **Language Proficiency Testing:** Any Humana employee who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.$63,400 - $87,400 per yearThis job is eligible for a bonus incentive plan.
