The Assistant Director is responsible for the implementation of a strategic marketing and outreach plan by product line, including long-term, sub-acute, pediatric, acute rehab and TBI services. The Assistant Director will develop relationships with new referral sources and maintain positive relationships with existing referral sources. Through the building of long term relationships with these referral sources, the Assistant Director will facilitate the identification and assessment of future patient/resident admissions factoring in length of stay and level of acuity which should align with the outcomes expected from Kingsbrook’s clinical expertise in rehabilitation and nursing.

Responsibilities

Assists in the coordination and implementation of all facility admissions, outreach and marketing activities for hospital and SNF referrals and the coordination of the admission of residents & patients, including long-term, sub-acute, pediatric, acute rehab and TBI, in a manner that is supportive of the facility’s mission and values.Demonstrates knowledge of admission criteria for all product lines.Responsible for the daily monitoring of the census for all long-term, subacute, pediatric, acute rehab and TBI beds.Performs the assessment of referral, planning, implementation and evaluation of patient care; demonstrates competence in assessing physical, psychological and social dimensions of patients.Collaborates and communicates with Kingsbrook’s Admissions Departments (both acute and SNF) to coordinate transfers from facilities and achieve census and revenue targets.Coordinates patient/resident navigation through KJMC system in concert with the transitions of care staff.Assists in overseeing the Outreach & Marketing team, including productivity of each member of the department.Collaborates with discharge planners regarding anticipated discharges from facilities. Actively assists the referring and accepting institutions in addressing requirements of specific patients.Develops relationships with hospitals, nursing homes and community based organizations. Ensures all hospital partners are educated on the services provided by the facility on an ongoing basis.Provides patient follow-up to discharge planners to provide value add.Prepares and delivers presentations to promote the goals of the organization. This includes attendance at community activities, as well as outreach at churches, synagogues and senior centers.Serves as an educational resource for patients, families, hospitals and healthcare professionals.Develops relationships with third party payors and has a working knowledge of insurance verification process.Identifies quality issues and participates in quality management activities, as needed.

Qualifications

Education:Master's degree in Nursing or Social WorkExperience:One to two years experience in marketing & outreach in an acute care and/or a skilled nursing facility setting.Demonstrated success in marketing and outreach and enhancing occupancy.Working knowledge of Medicare beneficiary coverage, ECINS, Allscripts and MediLinks applications.Licenses, Certifications:NYS Licensed Registered Nurse preferred, Licensed Master Social Worker (LMSW), or Licensed Clinical Social Worker (LCSW)Knowledge and Skills: .Ability to hear, talk, walk, stand, bend and stretchAbility to read, write and interpret documentsAbility to communicate effectively with patients and/or co-workersMust be trained in and knowledgeable of all Kingsbrook/Rutland policies and procedures pertaining to hazardous waste and hazardous materials and maintain qualifications applicable to assigned workOther: Knowledge of all codes and demonstrate understandingPhysical Requirements:Normal routine involves no exposure to blood, body fluid or tissue.
