They also experience healthier work-life balance with the flexibility that comes with hospice care for patients in their homes.About HospiceIn collaboration with the entire interdisciplinary team, assess, plan, implement, coordinate, monitor and evaluate the strategies and services required to meet a patient/family's end-of-life care needs. An outcome-based plan of care is created and continually adjusted through utilization of the nursing process, relationship centered care principles and holistic philosophy assuring the changing needs of the patient/family are met while facilitating a symptom-free, peaceful, sacred death.Scope of workAssumes responsibility for evaluating client's need for music therapy. Plans and executes treatment program in accordance with guidelines outlines by the American Music Therapy Association (AMTA). Maintains skills through continuing education. Serves as a clinical resource to peers, team members and client/family. Participates in quality improvement and program development activities. Identifies clients who are appropriate to receive and will benefit from music therapy services. Conducts and documents music therapy assessments. Develops initial and ongoing music therapy treatment strategies involving the client, family members, and other clinicians and staff members as appropriate. Establishes long and short term treatment goals.
