Job SummaryThe Center for Excellence in Primary Care (CEPC) housed within the Department of Family and Community Medicine (DFCM) at Zuckerberg San Francisco General Hospital (ZSFG) is seeking an exceptional Trainer and Communications Coordinator who is passionate about improving healthcare delivery, particularly for the underserved. As a Trainer and Communications Coordinator at CEPC, you would share in our mission to identify, develop, test, and spread promising innovations in primary care to improve the patient experience, enhance population health and health equity, reduce the cost of care, and restore joy and satisfaction in the practice of primary care. The Trainer and Communications Coordinator will be working in partnership with clinicians, clinic staff, and trainees who have dedicated their careers to advancing health equity and delivering compassionate, world-class care to the most vulnerable populations.Primary care transformation requires the implementation of essential components of high performing primary care, such as team-based care and data-driven improvement. CEPC calls these the Building Blocks of High Performing Primary Care ( http://www.chcf.org/publications/2012/04/building-blocks-primary-care ). Drawn from the experiences of high performing primary care practice, the building blocks provide a roadmap to improvements in patient experience, population health, and the sustainability of primary care. CEPC has a unique opportunity to help primary care health centers, both within the safety net for San Francisco and more broadly across the country, integrate these Building Blocks into their practices through training to clinics seeking to build teams to meet the needs of their population through models such as panel management and complex care management.As a Trainer this staff member will co-facilitate trainings and learning sessions alongside CEPC master trainers on CEPC's standard curricula, which includes health coaching, panel management, and practice coaching. Health coaching is a patient-centered approach to delivering care that helps patients build the knowledge, skills, and confidence required to manage their chronic conditions and improve their health. Health coaches empower patients to play a central role in clinical encounters and to engage in self-management activities at home, work, and schools, where they spend most of their lives. Panel management is a proactive way to ensure that everyone assigned to a clinic is up to date on basic preventive care - like cancer screenings or immunizations - and that they receive extra help if they have lab numbers that are high. Medical assistants, health workers, and nurses play a critical role in providing panel management. Practice Coaching (also called Practice Facilitating) is a way to assist primary care practices and clinics to improve the way they deliver care. CEPC's practice coach training is built around the 10 Building Blocks of High Performing Care model that was developed at CEPC. The Trainer and Communications Coordinator will also assist with development of new curricular materials, revision of existing materials, and developing, revising and tailoring training materials.As a Communications Coordinator for CEPC's Training programs, this staff member will provide support for a broader host of CEPC training services by managing recruitment, registration, evaluation, logistics, contracts, vendor relations, customer service, and travel planning and reimbursement for the training team. This staff member will coordinate communications for the CEPC website, newsletters, and social media. The Trainer and Communications Coordinator will be involved in strategic planning for training programs and development of communication strategies via social media, website, and listservs.They will also provide support for other CEPC programs as needed.Required QualificationsBachelor's degree in related area and one to three years of directly related experience; and / or equivalent experience / training.Demonstrated skills in designing trainings that apply adult learning theories and education methodologiesDemonstrated knowledge of the CEPC's model of practice improvement - the 10 Building Blocks of High Performing Primary CareTechnical - familiar with various items of audio-visual equipment (including use of PowerPoint, laptop projection, LCD projectors, audience response systems, etc); skills to manage and distribute electronic information (documents, spreadsheets, Web links, etc) across a variety of platforms (PC, shared servers, Macintosh), using various modalities (ASCII, PDF, Word, Excel, PowerPoint, HTML); skilled at converting PowerPoint and Keynote files into PDF documents; posting and downloading files from a web server and accessing file sharing programs, such as Dropbox.Working knowledge of functional area and understanding of how training may impact other areas.Familiar with current training programs and trends.Good skills in delivering training programs.Good verbal, written and interpersonal communication skills and good presentation skillsPreferred QualificationsAt least 6 months of experience related to health coaching, which may include direct provision of coaching to patients or delivering training programs to health coaches.At least six months of direct experience in practice improvement, including faciliating meetings among clinical staff to identify improvement goals, assisting with design of new initiatives, identifying metrics to measure success, and helping staff to review metrics and propose modifications to improvement initiatives.Demonstrated knowledge of practice improvement principles and methods such as Plan-Do-Study-Act cycles and run charts.At least three months of experience designing and delivering interactive, skills-based trainings to healthcare professionals.At least six months of experience working in a primary care clinic.At least six months of experience using an electronic health record or chronic disease registry, such as the eClinical Works, Apex/Epic, or i2i Tracks.Event and Meeting Planning - sufficient understanding of meeting planning processes -- including hotel operations, meeting specifications, food and beverage planning, travel arrangements and audiovisual specifications.Financial - knowledge of UC Purchasing policies (delegated budget amount, preferred vendors, Bear Buy, P-card), UC Accounting procedures (daily spending amounts, My Expense), and Departmental policies (spending limits, expense delegation limits, preferred vendors, insurance coverage, financial liabilities, etc)Working knowledge of delivering training programs and of other areas of human resources.Working knowledge of conducting needs assessment.Good analytical skills to understand how business needs can be addressed through the design and delivery of training programs.Working knowledge of organizational policies and procedures.Working knowledge of adult learning theories and education methodologies.About UCSFThe University of California, San Francisco (UCSF) is a leading university dedicated to promoting health worldwide through advanced biomedical research, graduate-level education in the life sciences and health professions, and excellence in patient care.
