Peninsula Agency on Aging (PAA) is currently seeking an individual to serve as the Options Counselor for the Veteran’s Directed Care (VDC) Program to provide case management and service coordination for eligible Veterans.IntroductionPeninsula Agency on Aging has been providing services that have enabled older adults to live independently since 1974. Our mission is to support the independence and quality of life of Peninsula residents, primarily 60 years and over, by advocating, arranging, or providing vital human services.Our staff value and model empathy and compassion, reliability, ability, respect for all, and communicating with kindness.Key ResponsibilitiesAs the Options Counselor for the VDC Program, you will be instrumental in advising Veterans and/or their caregivers of the services and resources available to support aging in place. You will utilize interview skills to screen and complete assessments, provide options counseling, and develop comprehensive plans of care/service plans for Veterans. This position may require significant travel. An agency vehicle is available, but staff may also be required to utilize a personal vehicle for travel. Mileage is reimbursed at the IRS mileage reimbursement rate.Essential Skills and ExperienceWe are looking for applicants with any combination of education and experience equivalent to a Bachelor’s Degree from an accredited college or university in social work, human services, or a related field. Applicants need to be experienced with working with seniors and/or disabled adults, case management, excellent interviewing, oral and written communication skills, and effective use of computers, software, and mobile technology. Applicants must also have the ability to model our values and be vaccinated for COVID.In addition to our highly accessible central location in Newport News, Peninsula Agency on Aging offers a competitive hourly rate, medical, dental, and vision coverage, vacation and sick leave, agency paid life insurance, 403b plans, paid holidays, and an inclusive work environment that is flexible, friendly, positive, and rewarding.Full-time, M-F, 8:30 am – 5:00 pm, 40 hours/week, 1 day remote.
