Your key responsibilities will include collaborating with local jails, community agencies, and program participants to eliminate fragmentation and gaps in care. You will provide behavioral health consultations, screenings, and targeted intervention strategies, ensuring that clients receive the support they need upon release.Your expertise in clinical assessment and understanding of community resources will enable you to deliver direct short-term services to a small caseload of consumers as they reintegrate. Additionally, you'll work closely with the Forensic Services Team to address complex needs related to co-occurring disorders and other challenges. Your ability to build rapport with clients and maintain accurate records within electronic health systems will be essential to your success and that of the clients you serve.What you need to be successfulTo excel as a Forensic Discharge Planner at Hampton-Newport News Community Services Board, candidates must possess a Bachelor's Degree in Human Services and at least three years of direct care experience focusing on case management and counseling for individuals with Serious Mental Illness (SMI). A strong background in coordinating health care plans with multiple community agencies is essential, along with proven skills in behavioral health consultation and crisis intervention. Forensic experience is preferred, as it enhances your ability to navigate the complexities of this specialized population. Additionally, being a Qualified Mental Health Professional (QMHP-A) will set you apart in delivering comprehensive care. A valid driver's license is required to facilitate client appointments and community outreach.The ideal candidate will demonstrate excellent communication skills, problem-solving abilities, and the capacity to build rapport with clients, ensuring effective engagement throughout the recovery process.Knowledge and skills required for the position are: Bachelor's Degree in Human Services and three (3) years of case management and counseling services to the client population assigned; individuals with Serious mental Illness (SMI). Experience working with multiple community agencies coordinating health care plans and interventions. Forensic experience preferred. Qualified Mental Health Professional (QMHP-A) Preferred.
