We provide exceptional homecare and hospice services in the comfort of one's home or assisted living facilities.Position Summary: Chelsea Jewish Lifecare is seeking a Part- Time Music Therapist to work in our VNA/Hospice division to travel throughout our community. The ideal candidates should be able to practice independently, yet embrace the importance of the Team in provision of our services. We value individualized, patient-centered care within our highly skilled, supportive care team.REQUIREMENTS: Demonstrates skill in utilizing music, vocal and instrumental, to create therapeutic response from individuals and/or groups. Ability to evaluate needs and response to music interventions. Possesses excellent interpersonal skills with patients/families and staff/community members. Is self directed, able to function independently, yet work within the organizational framework.PURPOSE OF POSITION: Provides music therapy for patients and families as a part of comprehensive care provided by Legacy Lifecare Hospice and Palliative Care.FUNCTIONS:•Visits patients/families to offer therapeutic music activity.•Provides music activity (vocal/instrumental) in one-to-one settings and/or in group settings.•Performs assessment of the needs of patients/families and documents on "Music Therapy Assessment".•Documents activity and patient/family response in "Music Therapy Assessment".•Develops plan to meet individual needs, considering the cultural, religious and personal needs/preferences.•Interacts with members of IDG team to report patient/family response to music activity. Receives requests, suggestions and/or observations from the team to enhance the therapeutic value provided by the music interventions.•Leads or participates in group activities in facilities where Hospice patients reside.Chelsea Jewish Lifecare conducts extensive background checks and professional references for all potential employees.
