Job Responsibilities:

As a Music Therapist at Custom Home Health, you'll be at the forefront of providing tailored care to patients and their families in the comfort of their homes. Your duties will include:

  • Conducting thorough assessments of music therapy needs for patients and their families
  • Collaborating with interdisciplinary teams to prioritize interventions and referrals
  • Meticulously documenting all care provided in adherence to hospice documentation standards
  • Educating hospice staff and the community on the impactful role of Music Therapy
  • Offering bereavement support to families when deemed appropriate
  • Facilitating support groups as needed and ensuring efficient resource utilization
  • Fulfilling additional duties as assigned with enthusiasm and dedication

Preferred Qualifications:

  • Bachelor's degree or equivalent in Music Therapy
  • State licensure as a Board-Certified Music Therapist (MT-BC)
  • A minimum of two years' experience in music therapy, with a preference for community/hospice experience
  • Deep understanding of hospice philosophy, focusing on comfort and maintaining care in the patient's residence for as long as possible
