1. Complete the music therapy assessment with patient and family when referred in accordance with the interdisciplinary plan of care.
2. Plan music therapy interventions.
3. Implement the music therapy interventions in accordance with the interdisciplinary plan of care.
4. Guide the patient in the use of creative therapeutic activities for the purpose of improving or maintaining mental and physical function and quality of life.
5. Instruct other care team members including, when appropriate, hospice aides and family members in the use of music to achieve individualized care plan goals.
6. Evaluate the effectiveness of music therapy services with the patient and/or family-of-choice.
7. Document all services provided to hospice patients and families in the clinical record.
8. Terminate therapy services when appropriate.
9. Provide in-services to staff, hospitals, nursing homes, and other facilities as requested and determined appropriate by therapist.
10. As one of the counselors in the interdisciplinary hospice group (IDG), the MT serves as the counselor at IDG meetings, participates in on-call rotation, and assists with sign ups and consents for newly referred patients.
11. Participate in the on-call schedule as required.
12. Support the bereavement program at the site level, including, but not limited to:
* Completing bereavement calls and visits to offer counseling and support;
* Facilitating bereavement support groups in the community, including an annual memorial service;
* Participating in the monthly bereavement committee meetings;
* Keeping the bereavement binder up-to-date (with meeting minutes, sign-in sheets from groups/workshops, etc.).
13. Performs other duties as assigned including those within the scope of current, valid professional licensure/credentials (i.e. RN, SW, Chaplain, MT, HA, etc.). Additional Job Functions for a Level II Music Therapist:
* Provide supervision of interns (as long as AMTA minimum requirements are met).
* Participate on a committee within the organization.
* Be a support to community outreach and education activities (in partnership with the Volunteer and Business Development departments), as appropriate.
* Facilitate bereavement groups or workshops and memorials.
Additional Job Functions for a Level III Music Therapist:
* Provide supervision of interns (as long as AMTA minimum requirements are met).
* Participate and chair a committee within the organization, including ethics consultations.
* Participate in or submit an abstract annually in an external Music Therapy Committee (such as AMTA, CBMT, or a regional MT Association) or to a Hospice & Palliative Care Conference (such as NHPCO, a State Hospice & Palliative Care Association, ADEC, NAGC, etc.)
* Be a support to community outreach and education activities (in partnership with the Volunteer and Business Development departments), as appropriate.
* Facilitate bereavement groups or workshops and memorials.
