Role Summary:
As a Home Health Care Liaison, you will establish and maintain effective relationships and communication channels between the agency, medical community, and the public. You will be a results-oriented marketer with strong interpersonal skills, a passion for healthcare, and a proven track record of success. You will develop and execute innovative marketing strategies to drive growth and increase patient referrals.

 

Core Responsibilities:
• Outreach & Networking: Partner with hospitals, physicians, and the community to identify needs and strengthen connections.
• Marketing & Education: Organize health fairs, public events, and provide education to increase awareness of agency services.
• Administrative Support: Manage physician orders, patient forms (485s), and facilitate information flow between parties.
• Trend Awareness: Stay informed about telehealth, in-home testing, and Medicare regulations.

 

Qualifications:
• Strong interpersonal and communication skills.
• Knowledge of Medicare Part A & B.
• Experience in healthcare marketing and community engagement.
• Commitment to confidentiality and secure data handling.
• Must have knowledge of The Greater Houston Area and Book of business (Medicare Beneficiaries)
