Were looking for a Community Outreach Specialist who is passionate about community outreach for our PAS program.

Your Role:

• Identify potential patients/clients who can benefit from our services.
• Engage with patients/clients, their families, caregivers, and community members to ensure they are satisfied with our care.
• Develop and implement programs to enhance our community outreach efforts.
• Participate in community events, health fairs, and clinics to raise awareness.
• Build strong relationships with local doctors offices, community centers, senior living facilities, and programs.

What Were Looking For:

• Three years of customer service experience (preferred).
• One year of experience in marketing or community outreach (preferred).
• Comfortable working with Medicaid-qualified clients and senior care facilities.
• Reliable transportation and the ability to use your personal vehicle for field visits (mileage reimbursement provided).
• Bilingual in English and Spanish (required).
