Responsibilities

The RN (Registered Nurse) Discharge Expeditor is responsible for researching anticipated discharges and assessing patient and family needs prior to discharge. Assist direct care nursing staff to focus on addressing discharge needs and escalating discharge barriers.

Qualifications

Education

• Bachelor's Degree in Nursing required

• High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate AND an additional 4 years of experience in an acute care setting may be substituted for the Bachelor's Degree

Experience

• 1 year of experience in an acute care setting required

• Case management experience preferred

Licenses and Certifications

• RN - Current and valid Registered Nurse license to work within the state of California required

• BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
