Job Description:
As a Home Health Care Community Liaison, you will play a crucial role in connecting our organization with community resources, building relationships with referral sources, and promoting awareness of our services. You will serve as a liaison between our agency and various community organizations, healthcare providers, and potential clients to ensure seamless coordination of care and exceptional service delivery.

Responsibilities:

  • Develop and maintain relationships with healthcare professionals, hospitals, nursing homes, and other community organizations to promote our services and increase referrals.
  • Educate healthcare professionals and community members about the range of home health care services we offer and how they can benefit their patients or clients.
  • Participate in community events, health fairs, and other networking opportunities to raise awareness of our agency and establish our presence in the community.
  • Collaborate with our clinical team to ensure smooth transitions of care and continuity of services for our clients.
  • Conduct assessments of potential clients and their homes to determine their needs and eligibility for our services.
  • Provide support and assistance to clients and their families in navigating the healthcare system and accessing community resources.

Qualifications:

  • Previous experience in sales, marketing, or community outreach preferred.
  • Strong communication, interpersonal, and networking skills.
  • Ability to work independently and as part of a team, with excellent organizational and time management skills.
  • Valid driver's license and reliable transportation.
