Most employees should expect onsite requirements and at a minimum of once per month.This position is remote/hybrid within the state of AZ onlyPurpose of the Job The Health Promotion Coordinator is responsible for tasks related to increasing compliance with performance measures and promoting comprehensive health care for members from birth to age 21. Qualifications REQUIRED QUALIFICATIONS Required Work Experience 1 year of experience in health insurance, or other health care related fieldRequired Education High-School Diploma or GED in general field of studyRequired Licenses N/ARequired Certifications N/APREFERRED QUALIFICATIONS Preferred Work Experience 1 year of experience in a managed care organization or related healthcare delivery systemPreferred Education N/APreferred Licenses N/APreferred Certifications N/AESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES Enter information from the tracking forms into the Medical Management SystemProcess ReferralsForward specific referrals to appropriate departmentsMaintain database and documentationEnter and update data in the Immunization databaseOutreach made to the parent/guardian of members whose immunizations are not up-to-dateContact members via telephone and/or mail on child health and wellness related topicsCommunicate with providers on member related issuesFollow up calls are made to members who do not show to their scheduled doctor visit to identify possible obstacles to attending scheduled visitsOutreach to members referred to services to verify that needs have been metPerform all other duties as assignedThe position requires a full-time work schedule.
