Now Offering a $7,500 Sign On Bonus 7 day on, 7 day off schedule - great for work life balance. Reports directly to the Administrator, Executive Director or Patient Care Manager Senior and will maintain primary control and professional management of each patient. You will act as primary liaison between physician, patient/family and hospice team.
