United Hospice Inc., is a community-based, not-for-profit hospice, that provides care and support to thousands of your neighbors, colleagues and friends in Rockland and Orange County, New York.
United Hospice Inc., envisions a community in which all individuals and their loved ones facing serious illness retain their dignity and hope while receiving the best care and support that hospice has to offer. We are currently seeking to fill the following position:
Location: Rockland & Orange Counties
Schedule: Per Diem, as needed
May Require Weekends as needed
Job Duties:
Music Therapist evaluates benefit of music therapy for all referred patients and families.
- Provides direct services to patients and families: Develops a music therapy treatment plan based upon the physical, spiritual and psychosocial assessment of patient/family.
- Provides music therapy in patient’s place of residence to include individual and family intervention.
- Provides support to patients and families around issues involving death and dying.
- Participates in patient/family related Hospice sponsored special events.
- Procures all equipment/resources necessary for provision of music therapy
- Provides timely documentation of all interventions for patient record.
- Provides orientation/education of staff/students/volunteers to music therapy as needed.
- Music Therapist maintains professional expertise through participation in continuing education.
- Represents Hospice to community agencies, through meetings, presentations and other appropriate activities.
- Consults with other disciplines to share knowledge of patient/family, and social situation to enhance interdisciplinary care plan.
- Provides music therapy services as part of the bereavement program
- Music Therapist participates in QAPI program and hospice sponsored in-service training.
- Performs other responsibilities as assigned.
Qualifications:
College Degree in Music Therapy from an accredited college or university.
- Certification from the American Music Therapy Association.
- Minimum one year hospice or related experience.
- Demonstrates the emotional maturity and sensitivity to work with the terminally ill.
- Must have ability to provide own transportation for the purpose of making home visits and other business related activities.
The Music Therapist position is per diem, (as needed) and is not benefits eligible.
