Intensive and Sustained Engagement Team (INSET)

INSET offers mobile, community-based, flexible and sustained interventions to support individuals who have frequently faced periods of what has been described as acute distress, recurrent emergency room visits and hospitalizations, and for whom prior programs of care and support have been ineffective or harmful. This peer-led, person-first approach looks beyond assessing an individual’s symptoms to address their immediate needs while also establishing and/or strengthening family and social networks for sustained support to help participants achieve their specific goals through unique care coordination without traditional
limitations on time and physical space requirements. The INSET Family Liaison functions as an advocate and offers to support the INSET participant and/or their family (of choice) if indicated by the participant. The Family Liaison will offer services such as advocacy, connecting families with services and referral, and one-on-one support that reflects the rights of self-determination, choice, shared decision-making, and collaboration from a trauma-informed and culturally curious framework. SPECIFIC DUTIES AND RESPONSIBILITIES : Establish and maintain contact with chosen families of INSET participants or individuals referred to INSET. Support the families in accessing appropriate services based on indicated needs. Advocate with and for families, coaching them on shared decision-making. Meet with families in person or virtually. Support families in strengthening their caregiving skills. Guide families in enhancing their natural support system and increasing connections with their community. Contact community agencies to explore possibilities for program expansion to better serve the needs of families. Use computer applications or other automated systems to document progress and maintain a record of support. Other duties assigned as needed. REQUIRED KNOWLEDGE AND SKILLS : The Family Liaison must have knowledge of mental health diagnosis, suicide assessment, and crisis intervention techniques. The Family Liaison must have knowledge of mental health resources and services and other health, financial, social, recreational, cultural, housing, and legal advocacy services. Highly organized and self-motivated. Must be able to perform duties in an independent and efficient manner. Must be able to effectively engage clients. Familiarity with and ability to use an Electronic Health Record system. Must complete all training and steps toward FPA credential and maintain FPA credential. QUALIFICATIONS : Family Liaison workers must: Be at least 18 years of age. Have a high school diploma or equivalent. 2 years of related work experience or a Bachelor’s degree in a related field. Demonstrate “lived experience” as a parent or primary caregiver who has navigated multiple child-serving systems on behalf of their child(ren) with social, emotional, developmental, health, and/or behavioral healthcare needs. Holds a NYS Family Peer Advocate (FPA) credential or be eligible to obtain one within the first 6 months of hiring, with the expectation of obtaining the full credential within 18 months of commencing employment. Basic computer knowledge is required. Must be reliable and able to work independently and understand the importance of maintaining confidentiality. The Family Liaison must have a valid Driver’s License and a driving record that is satisfactory to our insurance carrier. The Family Liaison must maintain current New York State Defensive Driving certification. The Family Liaison must be cleared by the NYS Justice Center through fingerprinting for a Criminal History Records search. The Family Liaison must be cleared by the NYS Office of Children and Family Services for instances of child abuse and/or neglect.
