We currently have an opportunity for a Clinical Liaison/Communications Specialist.
The Clinical Liaison/Communications Specialist (CCS) will serve as a vital link amongst the treatment team disciplines. The role will focus on early identifications of discharge challenges, coordination of Clinical Services outcome assessments, and daily communication with family members to provide updates and address concerns. The CCS works collaboratively with the discharge planner to ensure seamless transitions and continuity of care.
Job Duties/Responsibilities:
- Daily communication with families of patients aged 18-26.
- Clinical Services evaluations and assessments. BASIS/BPRS/PDRS assessments with new patients within 24 hours and every seven days of their stay.
- Complete Health-Related Social Needs Screening tool within 24 hours of admission, documenting the findings on shared spreadsheet.
- Discharge planning support.
- Identifying discharge issues starting as early as day two and documenting on a shared spreadsheet.
- Daily follow-up with the discharge planner on those that have non-traditional discharges, providing necessary updates to all involved parties.
- Team Collaboration.
- Acts as liaison between Clinical Services and the Discharge Planner.
- Facilitates meetings as needed.
- Other support duties as assigned.

Qualifications
Clinical Liaison/Communications Specialist Job Requirements:
- High School Diploma or GED, some college in related field preferred.
- Minimum two (2) years healthcare, behavioral therapy, or social services setting.
- Excellent verbal and written communication skills.
- Ability to work collaboratively within a multidisciplinary team.
- Empathy and sensitivity to the needs of patients and their families.
- Licensed or certified in relevant field is preferred.
