Reporting to the Hub Manager, provides patient flow and outcome management services in collaboration with Northwest Community Healthcare (NCH) inter-disciplinary teams to meet the clinical care needs of patients. Collaborates with the healthcare team to achieve optimal efficiency in clinical, financial, length of stay and satisfaction outcomes. Monitors and facilitates patient flow and assists the care team in evaluating options, services and interventions to meet patient's health care needs and length of stay targets through effective communication, collaboration and coordination required for efficient clinical care progression.
What you will need:
Bachelor's of Science in Nursing requiredCurrent licensure to practice nursing in the State of IllinoisMinimum two years of experience in an acute care setting requiredPrevious leadership experience required
Overall Qualifications:
Advanced Cardiovascular Life Support (ACLS) - American Heart Association (AHA), Basic Life Support (BLS) - American Heart Association (AHA), NIHSS Stroke Scale Certification (NIHSS) - National Institute of Health Stroke Scale (NIHSS)Associates Degree: Nursing (Required), Bachelors Degree: NursingCare Management / Patient Care Coordination, General Nursing, Neurology, Patient Access
