The purpose of this position is to establish an effective relationship with hospitals and the community that enhances the facility's visibility in the market. This position serves as a liaison between hospital case management/discharge planners and the facility to improve the admission process and increase referral volume.
Some of the duties are:
Nurtures referral relationships;
Develops and maintains referral base;
Facilitates referral response times, admission decisions, and ensures a positive admission experience for customers and families;
Meets census goals by working with the facility teams and Network entities to coordinate patient flow into and through the facility;
Ensures an immediate response to phone, fax, and walk-in inquiries and referrals;
Coordinates tour, application, and sales process with customers, families, and referrers, either personally or with appropriate backup systems;
Communicates admission criteria to referral sources, customers, and families;
Conducts follow-ups on all inquiries which have not yet been converted to admissions;
Works with the Executive Director and appropriate staff to ensure consistent, effective admissions, discharge, and transfer meetings;
Maintains statistics indicating customer flow, pending and lost inquiries, denials, hospital discharges, and marketing activity; submits to corporate office;
Sets and coordinates healthcare executive-level meetings;
Works to initiate, nurture, and maintain contacts with physicians, social workers, and discharge planners as valued customers to maximize referrals and facilitate admissions;
Stays up to date with services available within the facility and through the Network in order to serve as a liaison with the facility to hospital discharge planners, utilization review nurses, physicians, and other professionals to assist in the efficient use of the center and alternate care settings within the Network;
Provides information to referral sources of customers' progress as appropriate;
Identifies and facilitates marketing opportunities for referring hospitals and physicians, as it relates to the individual center as well as center clusters; works with Administrator and Regional marketing to follow-up, as needed;
Develops and implements long-range and short-term marketing plans;
Experience:
Skilled and Long-Term Care Nursing Home experience of 2 years

$85,000.00 - $95,000.00 per year
