Are you outgoing, enthusiastic and love working with people? We are searching for a compassionate and energetic self-starter to become our new Admission Coordinator/Community Marketer who can proudly share our patient care vision with prospective residents, families, community partners and referral sources. This position will act as a facility liaison with patients, hospital discharge planners, physicians and community healthcare partners.

Responsibilities and Purpose

  • Develop and implement a community marketing plan, focusing on senior healthcare partners.
  • Outside marketing, maintaining established referral relationships with physicians, discharge planners and community referral sources.
  • Primary contact for all telephone admission inquiries, walk-in admission inquiries and request for pre-admission facility tours.
  • Directly responsible and accountable for the completion of all admission forms and facility notification documentation.
  • Coordination and regular communication with the corporate intake team and other referral sources, ensuring a smooth patient discharge and admission experience.
Desired Experience and Education
  • Experience marketing and developing relationships within the senior healthcare environment.
  • Knowledge of Skilled Nursing, Rehab and Long Term Care services.
  • Working knowledge of Medicaid, Medicare and third party managed care payer sources.
  • Basic computer skills, including Microsoft Office.
  • Social Services and/or nursing experience a plus.
