SUMMARY:
The Health Promotion Specialist is responsible for providing community engagement, patient recruitment, general health education with emphasis on contraceptives as well as HIV/STI related prevention services including HIV Testing. The Health Promotion Specialist works in clinic and community environments as part of a Mobile Unit team to seek individuals in need of prevention services and link them to Spring Branch Community Health Center for care.
QUALIFICATIONS:
- High School Diploma or GED.
- Certified Community Health Worker or Certified Medical Assistant
- Knowledge of Federally Qualified Community Health Centers (FQHCs), Mental Health/Substance Use Disorders (SUDs) community resources, and housing-related providers as well as services.
- Must be comfortable and culturally competent to work with special populations such as LGBTQI+, People Experiencing Homelessness, and/or People Experiencing Behavioral Health Issues or Concerns.
- At least two years of paid experience providing HIV testing services to individuals at high risk for acquiring HIV/STI as well as individuals newly diagnosed with HIV/STI.
- Knowledgeable about contraceptive care practices and experienced in safer sex education.
- Experience must include providing peer counseling, support, harm reduction, education and/or advocacy services.
- Bilingual- English/Spanish is required.
- Reliable transportation to and from SBCHC clinics.
- Valid Texas Driver’s License and personal automobile insurance.
- Must pass pre-employment background check and drug test.
- Lived experience preferred.
- Region: Spring Branch, outreach to other clinic regions required
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Provides targeted outreach and recruitment including conducting HIV testing (e.g. venipuncture, Insti, Orasure, Oraquick), contraceptive counseling and sex education.
- Provide education and warm handoffs to clinic contraceptive care services during community events and safer sex practices in order to prevent HIV/STI infection or re-infection.
- Educate on accessing PrEP and ART services in collaboration with mobile unit team.
- Develop and assist in the development of health education materials such as curriculum, workshops, pamphlets and bulletins, and publicize and interpret health services and programs.
- Provides general health education/Evidenced Based Interventions (EBIs) in English and Spanish (i.e.HIV/STI prevention, contraceptive education, chronic health conditions).
- Communicates with Mobile team and clinic staff to provide or exchange necessary information to execute warm handoffs to the clinics or community partners for service linkage.
- Accurately document all services and activities provided and as needed for program evaluation
- Participate in community engagement and outreach activities to promote Health Center services and provide information about other services in the community.
- Must have flexible schedule to conduct classes and attend community events after business hours and on weekends as needed.
- Drive as needed company vehicle/Mobile Unit for testing events.
