Concho Hearts Hospice is looking for an experienced Hospice Marketer in Beaumont. The base salary is between $60,000 to $65,000 to start, with bonus opportunities and the possibility of a salary increase to up to $80,000 at a 6-month review.

Position Overview:

• The Hospice Community Liaison is responsible for establishing and maintaining public relations at facilities, hospitals, and other community businesses.
• The candidate must be self-motivated and knowledgeable in providing hospice education.
• The Hospice Community Liaison manages the process from referral to admission, which includes meeting with the patient and family to provide an overview of hospice services and obtaining consents for care.
• Concho Hearts Hospice is dedicated to providing compassionate care wherever you call home.
• Our team of Nurses, Aides, Chaplains, Social Workers, Volunteers, and Doctors provide symptom management and support for patients and families coping with end-of-life illnesses.

Responsibilities:

• Develop various channels of referrals through the selection, acquisition, retention, and growth of business partnerships with referral sources.
• Consistently visit, communicate, and monitor provider, community, customer, payer, and patient perceptions of Concho Hearts Hospice as a high-quality provider of services.
• Conduct educational in-services and community outreach to promote our superior hospice care services.
• Educate patients and their families about hospice expectations.
• Work closely with the clinical operations team to ensure seamless care transition coordination from an acute care setting to home.

Qualifications: MUST have Hospice Marketing experience of 2+ years.
