The Licensed Clinical Social Worker (LCSW) is a member of the Social Work Department located in the primary care offices to provide care management services to their patients. The LCSW is primarily responsible for face-to-face comprehensive biopsychosocial assessments, collaboration with the patient’s primary care physician to determine diagnosis and treatment through brief therapy, and continued follow-up with patients throughout the duration of the therapy process. This practice will consist of working with adult and geriatric patients who present with emotional disorders and mental illness in a primary care setting.

A LCSW works closely with the entire care team to provide optimal services to the patient. The LCSW demonstrates respect, dignity, kindness, and empathy in each encounter with all patients, families, visitors, and other employees regardless of cultural background. The LCSW strives to be a highly accessible position responsible for creating a positive impression with patients, physicians, and other visitors encountered, both in person and on the phone.

EDUCATION:

Minimum: MSW from an accredited school by the Council on Social Work Education

LICENSES AND CERTIFICATIONS:

  • KY - Licensed Clinical Social Worker (LCSW)
  • Class D Driver’s License

YEARS OF EXPERIENCE:

Minimum: 2+ years clinical social work experience in mental health.

REQUIRED SKILLS AND KNOWLEDGE:

  • Knowledge of the supervisory process.
  • Knowledge of emotional and mental disorders and the DSM.
  • Excellent communication skills and the ability to coordinate care well with an interdisciplinary team.
  • Ability to manage and prioritize multiple tasks.
  • Good organizational skills.
  • Ability to establish and maintain effective working relationships with patients, clinical employees, and the public.
  • Working knowledge of Excel, Windows, and Outlook, and the ability to learn other computer skills as needed.
  • Warm personality with concern for others.
  • Excellent verbal and written communication skills.
  • Excellent interpersonal skills.
  • Self-directed.
  • Demonstrated ability to work successfully in a team-based decision-making culture.
  • Demonstrated ability to work independently resulting in effective outcomes and on-time performance.
  • Experience in planning and coordinating multi-disciplinary communications strategies, strategic initiatives, and events.
  • Must respond and follow through to requests from customers promptly.
  • Must work carefully and precisely with attention to detail.
  • Must utilize resources wisely.
  • Performs duties willingly and with initiative, sharing necessary information so co-workers can do the same.
