Operating 24/7, we provide reliable, dignified care at home, specializing in health services, personal care, and champion services. Role Responsibilities Serve as the first point of contact for patients, families, and referral sources Market home health services and screen patients for CHHA program eligibility Facilitate smooth transitions from referral to discharge Generate leads, educate patients, and engage with hospitals, nursing homes, and case managers Maintain communication with patients, caregivers, and referral sources Handle administrative tasks including data entry, assembling intake packets, and scheduling appointments Address patient inquiries and concerns proactively Qualifications High school diploma required; college degree preferred Experience in intake or healthcare setting beneficial Marketing and lead generation skills Excellent communication skills Proficiency in Microsoft Office, especially Excel Ability to handle confidential information with integrity and professionalism Join Us If you're interested and believe you're a good fit, we look forward to meeting you!
