Job Summary:Under general supervision, the Health Promotion Specialist implements health promotion programs and provides direct client services.
Essential Functions (Major Duties or Responsibilities):
Plans, conducts, and evaluates health promotion programs.
Collaborates with other agencies for health promotion activities.
Represents the Department at community functions.
Prepares grant proposals.
Facilitates health promotion activities.
Performs program activities, including report writing.
Provides direct care services, assessments, and case management.
Monitors patient registry reports and provides follow-up care.
Collaborates with healthcare providers on quality assurance measures.
Knowledge, Skills, and Abilities:
Knowledge of health education principles and care management.
Effective communication and medical terminology knowledge.
Skills in medical case management and communication.
Ability to establish effective working relationships.
Education and Experience:Requires a bachelor’s degree and one year of relevant experience; or equivalent professional work experience. One year of work experience may substitute for education.
