Evidence Based Health Promotion Coordinator

Department of Health and Human Services (DHHS)

Pay Range: $59,696 - $69,888

Job Summary: The Evidence Based Health Promotion (EBHP) Coordinator is responsible for managing the Older Americans Act Title IIID evidence-based health promotion and disease prevention programming to advance the health and well-being of older adults. This position will develop community partnerships and resources to support evidence-based chronic disease prevention and health promotion programming.

Job Responsibilities:

  • Coordinate project partners who offer EBHP programs, ensuring compliance with program guidelines.
  • Manage and support the network of EBHP facilitators, including recruitment and training.
  • Oversee daily operations of the EBHP and Disease Prevention Programs, expanding partnerships for program availability.
  • Educate the community about Medicare Preventive Services and assist beneficiaries in learning about cost-saving programs.
  • Promote EBHP programs through outreach events and develop marketing strategies.
  • Research and evaluate additional EBHP programs for future implementation.
  • Staff regular meetings of the Aging Wellness Committee and serve as a liaison with local health consortiums.
  • Track program participation and provide regular reports and evaluations.
  • Perform other duties as assigned.

Minimum Qualifications:

  • Current resident of the State of Wisconsin.
  • Valid Wisconsin Driver's License.
  • Associate degree in Exercise Science, Kinesiology, Public Health, Nursing, or related field.
  • Two years of experience in a Health and Wellness role or working with older adults.
  • Intermediate knowledge of computer applications and data reporting systems.
  • Able and willing to travel 25% of work time.

Preferred Qualifications:

  • Knowledge of exercise and wellness self-management programs.
  • Knowledge of Medicare and Medicare Savings Programs.
  • Experience interacting with older adults and people with disabilities.
  • Bi-lingual in Spanish or Hmong.

Knowledge, Skills, and Abilities: Skills needed include customer focus, excellent communication skills, critical thinking, strong organizational skills, and the ability to work collaboratively. Ability to lift up to 20 lbs. and push/pull up to 10 lbs.
