As a Music Therapist at Custom Home Health, you'll be at the forefront of providing tailored care to patients and their families in the comfort of their homes. Your duties will include:
- Conducting thorough assessments of music therapy needs for patients and their families
- Collaborating with interdisciplinary teams to prioritize interventions and referrals
- Meticulously documenting all care provided in adherence to hospice documentation standards
- Educating hospice staff and the community on the impactful role of Music Therapy
- Offering bereavement support to families when deemed appropriate
- Facilitating support groups as needed and ensuring efficient resource utilization
- Fulfilling additional duties as assigned with enthusiasm and dedication
- Bachelor's degree or equivalent in Music Therapy
- State licensure as a Board-Certified Music Therapist (MT-BC)
- A minimum of two years' experience in music therapy, with a preference for community/hospice experience
- Deep understanding of hospice philosophy, focusing on comfort and maintaining care in the patient's residence for as long as possible
