Summary
The Health Promotion Specialist for the Tulane T-Cell Clinic will be responsible for implementing all Health Models activities including: policies on incentive storage and distribution, security and confidentiality, crisis management, and patient rights and responsibilities. The Health Promotion Specialist will meet with patients both on site as well as their preferred location for assessment and follow-up (must have personal transportation). The Health Promotion Specialist will work in support of and coordination with medical and non-medical case managers; in collaboration with Tulane T-Cell Clinic Director, Data Manager, and other relevant staff to ensure that needs are being met and program is achieving its stated goals of increasing the proportion of patients who are in care (linked or re-engaged) and once in care, achieve and/or maintain viral suppression. In addition, the Health Promotion Specialist will maintain updated knowledge of HIV medication management methods, participate in all steering committee conference calls and meetings to ensure coordination of care, and maintain progress notes and other required program documentation in coordination with case managers.
Required Knowledge, Skills, and Abilities
- Excellent organizational skills and ability to handle multiple tasks independently.
- Must have personal transportation.
- Proficient computer skills; excellent working knowledge of Microsoft Office software applications (Excel and Word).
- Experience with electronic medical records systems.
- Must possess a high capacity for problem solving with individuals who present for services with multiple behavioral health concerns.
- Knowledge of medical, substance use and behavioral health services within the Greater New Orleans area.
- Must work well independently and collaboratively to support staff and patients involved in the program.
- Excellent verbal and written communication skills.
- Ability to proactively work independently, handle multiple projects simultaneously; prioritize work, anticipate and meet established deadlines.
- Working knowledge of available community resources and aptitude in locating new services when needed.
- Ability to maintain confidentiality in all work performed.
- Ability to maintain organization in all work performed.
- Excellent interpersonal skills; ability to interact effectively and courteously with individuals of diverse cultural backgrounds.
- Ability to work collaboratively with diverse populations.
- Ability to work evenings and weekends as needed.
Required Education and/or Experience
High school diploma or equivalent and six years of directly-related work experience.
Preferred Qualifications
- BA or BS preferably in a social services field, i.e., public health, psychology, social work, criminal justice, etc.
- Experience with HIV care, research and/or program evaluation strongly preferred.
- Patient-centered/Patient-valued behavior.
- Ability to speak and write Spanish fluently.
Compensation Information
